Application Guidelines
All applications must comply with the following:
- Applications must not exceed 10 pages. Longer entries will not be considered.
- Type (no smaller font size than 10) or print clearly all information
on 8 1/2" x 11" paper.
- Place the name of the nominee on each response sheet.
- Attach a cover page to the front of your response pages that includes
the information outlined on the Nomination Application (Section A).
- Use a heading and/or corresponding numeral for each topic you respond
to. It is not necessary to use a separate sheet for each response.
- Ensure that if supporting documents are submitted, you reference them
in your written responses. Choose supporting documents carefully. (Must
not exceed 5 pages)
- Send your application and any supporting documents in one package by
registered mail, courier or through the website.
- Applications may be faxed, but must be followed by hard copy for the
sake of clarity.
- All submissions and supporting documents become the property of the
Awards of Excellence in Disability Management Program, cannot be returned
and may be publicized as part of the Awards program.
