Nomination Procedure
The National Awards of Excellence in Disability Management nominations
may be made by workers, labour organizations, employers, public and private
service providers and other stakeholders in the area of disability management.
If the nomination comes from within an organization, the nomination should
be submitted jointly (where applicable).
Nominations must be submitted according to the guidelines described in this application. Applications must be limited to 10 pages. Additional documents concerning the nominee will be accepted, but must not exceed 5 pages. This additional information should support the merits of the nominee, such as research outcomes, letters of endorsement, publication abstracts, mission statements and brochures. Longer entries will not be considered.
Nominees will be judged by the Evaluation Committee based on the information on the Nomination Application Form and any supporting documents submitted. The Evaluation Committee may request additional information to facilitate the awards decision. An audit will be conducted of the top finalists in each category.
Completed Nomination Applications may be returned to either of the addresses that follow.
